Find contact information such as phone number in the patient card, for when you urgently need to contact a patient.

The patient card has two states; folded and unfolded. The most important information about the patient, and the contact person if the patient is a child, is visible when it is folded.

Folded patient card shows:

  • name
  • identifier, such as SSN, NHS number, membership ID
  • sex assigned at birth
  • age
  • guardian and relationship to the patient, if relevant

Unfolded patient card shows:

  • date of birth
  • phone number (if added)
  • email (if added)
  • postcode , only relevant for units in UK
  • where patient is registered (if activated for the unit)
  • identifier, such as SSN, NHS number, membership ID – for the contact person
  • sex assigned at birth – for the contact person
  • age – for the contact person
  • phone number – for the contact person, if added
  • email, – for the contact person, if added

For units in UK – manage NHS number
If a patient is child cared for by a guardian, a caregiver can add and edit the child’s NHS number in the platform.
